As of the September 2026 CMS release, there are 12,326 Medicare-certified home health agencies in the 50 states and territories on this site. 8,074 have a quality of care rating, and 35% of those have 4 or 5 stars (average 3.2). California has the most, with 3,196.
Home health agencies send nurses and therapists to a patient's home after a hospital stay or during an illness. Medicare covers home health care when a doctor certifies that the patient needs skilled care and has trouble leaving home. CMS rates every certified agency on quality of care and, separately, on what patients said in a survey.
CMS's 1-5 star rating for quality of patient care, built from how often patients improved in walking, bathing and breathing, how quickly care started and how often patients went back into the hospital.

Ratings are relative: CMS sets the cut-offs so that each star level covers a share of home health agencies nationally.
Medicare Part A or Part B covers part-time skilled nursing and therapy at home when a doctor certifies that you need it and that leaving home takes considerable effort. There is no copay for the home health visits themselves; medical equipment is usually 20% after the Part B deductible. You can choose any Medicare-certified agency that serves your area.
