Ratings and measures come from the Centers for Medicare & Medicaid Services (CMS). The CMS star rating summarizes dozens of quality measures; the patient rating comes from the HCAHPS patient survey; ER time is the typical (median) time patients spent in the emergency department before going home; readmissions are the share of Medicare patients back in a hospital within 30 days. "Medicare safety penalty" means the hospital's payments were cut 1% this year under the Hospital-Acquired Condition Reduction Program.
